Chapel Haven’s Supported Living Program (SLP) helps adults live independently in the greater New Haven community—with the right level of support to succeed.

Designed for individuals transitioning from Chapel Haven programs and those joining us from the wider community, SLP offers personalized, a la carte supports guided by a collaborative team of staff, professionals, and family. Each person’s plan is tailored to their short- and long-term goals, promoting confidence, independence, and stability.
