C.A.R.E.S. Wraparound Program, their
family gained guidance, support, and a team
that helped them grow stronger, together.

When Elena and her two children, Sofia, age 11, and Mateo, age 7, joined St. John’s United Family Services, they were ready for a fresh start and a stronger path forward.

Through C.A.R.E.S., their family experienced a model built on Family voice and choice. Elena helped shape the vision. Sofia shared her goals for school and friendships. Mateo talked about what helped him feel calm and confident.

Their wrap-around team, including a Care Coordinator, Family Partner, and trusted community support organizations, came together to build a plan grounded in the family’s strengths.

Through Coordination, Advocacy, Resources, Education, and Support (C.A.R.E.S), the family gained tools to navigate school systems, strengthen communication at home, and build steady routines that helped everyone thrive.

Team meetings became a place of encouragement and collaboration. Progress was celebrated. Goals were adjusted as the children grew. 

The focus remained clear: keeping this family strong, connected, and rooted in their community.

Today, Sofia feels confident at school. Mateo is building positive friendships. Elena feels empowered and supported as a parent.

This is the power of wrap-around care,  family-centered, strengths-based, and community-driven.
