Application Process

Thank you for your interest in the FACTS program. To apply to the program, please complete the application and send the following materials to FACTS@icpnyc.org

  • A current resume outlining your educational and professional history
  • Three letters of recommendation prepared by professionals who can speak knowledgably about your clinical work, your responsiveness to supervision, your initiative and dependability (i.e. punctuality, attendance), etc. Letters should be sent under separate cover directly from the reference.
  • A copy of your graduate diploma
  • A copy of your license or limited permit. If you possess neither license, permit or limited permit, please explain whether you are in a position to attain either a license, limited permit or permit. Please note: all applicants to the FACTS program must be licensable. If you have any doubts as to whether you are currently licensable, please explain them in detail. Participants in the program must maintain a license, permit or limited permit for the entire term of their involvement with FACTS
  • A copy of your malpractice insurance (if applicable). If you do not yet have malpractice insurance but are accepted into the program, you will be required to obtain this insurance before the program begins.

There is an $85 application fee. 

  • Tuition for the 2025-2026 academic year is $3,650 for incoming candidates.
