Curtis V. Cooper Primary HealthCare, Inc. (CVCPHC) is a 501-c3 Not-For-Profit Federally Qualified Health Center that was established in 1974 by Curtis V. Cooper. The Health Center has provided comprehensive health care to individuals in Chatham County regardless of their race, ethnicity, economic background, or income for 52 years. Over the last 15 years CVCPHC has expanded access to care to reach more individuals within Chatham County and surrounding counties (Bryan and Effingham) by adding additional locations and utilizing mobile health units.
In 2025, CVCPHC served approximately 32,800 patients including Women, Men, and Children. Our services included Primary Care, OB/GYN, Behavioral Health, Podiatry, Dental, Lab, X-Ray, Ultrasound, Mammography, Pharmacy, Chronic Care Case Management, Vision, training site for medical, dental, other health care services, and Outreach & Enrollment. Also, we offer a sliding fee discount program to our patients, based on income and family size, to assist in reducing financial burdens. However, sometimes even with the discounts, patients cannot afford the nominal fee.
We invite you to join the fight to reduce financial healthcare barriers for our community. On December 12th, 2026, we will be hosting our 12th annual holiday gala in support of our Patient Assistance Financial Hardship Program and celebrate CVCPHC 52nd ANNIVERSARY of providing health care services. The funding received from our sponsors for this event will directly support patients who are experiencing financial hardships, including patients who are on high deductible plans. Most individuals who need assistance will contribute to receiving services for emergency dental services, critical ultrasounds, lifesaving medication, and maintenance medication to remain healthy. CVCPHC is determined to reduce the social determinants of health by offering this financial hardship program in conjunction with other patient assistance programs to promote the best quality health outcome.
Our efforts to minimize the burden for our patients would go even further with your organization being an official corporate sponsor. With your organization by our side at this upcoming event, we're sure to achieve this season's fundraising goal of $150,000.
