About This Event
Moving from pediatric to adult chronic condition care can be difficult. This program provides essential skills and resources you need for a smooth, healthy transition.
This program is FREE for pediatric patients with lupus and other chronic rheumatic conditions transitioning from pediatric to adult care and their families. Attendance includes program, lunch (pizza buffet), & gaming (football bowling, basketball connect 4, and cornhole).
Sessions: Program sessions are currently being developed cooperatively by LFA, WI Chapter with rheumatologists and medical professionals from Froedtert & the Medical College of Wisconsin, Children's Hospital of Wisconsin, UW-Health, & UW-Health Pediatrics, as well as lupus warriors who have gone through this transition. Stay tuned for more details!!
Post Workshop: Following the program and lunch, attendees can play on us!

Time: 10:30 AM - sign-in starts at 10 AM, Lunch at Noon, optional games to follow
Location: First And Bowl, 269 N 114th St, Wauwatosa, WI 53222 (near Hwy 41/45 & Burleigh)
Parking: There is free parking in their lot or on the street.
Entrance: Go in the main entrance and we will have a check-in table near by.
