Where mental health and primary care meet: Integrated Services

This discussion will review how new models of care, Certified Community Behavioral Health Clinics, and bi-directional integration of behavioral health and primary care services address health disparities experienced by individuals diagnosed with a mental illness. You will leave with an understanding of how these models provide patient-centered care to improve both physical and mental health outcomes.
