Demonstrate the value of using RxChange requests from pharmacists when facilitating medication changes, whether due to the potential for an adverse drug/allergic reaction or for cost savings. Identify patterns of RxChange utilization by pharmacists and prescribers to quantify the effect of RxChange on medication use and safety.

  1. Expand the use of RxChange.
    The results gathered from this study could demonstrate the value of RxChange messages as well as inform best practices for the implementation, use, and adoption of RxChange.
     
  2. Make medications more affordable.
    Switching to lower-cost medication alternatives as recommended by pharmacists can result in substantial savings for patients.
     
  3. Improve medication adherence.
    Efficient communication between pharmacists and prescribers when a medication change is needed due to adverse reaction potential or affordability concerns helps ensure patients take needed medications as prescribed and could reduce abandonment.

  1. Eliminate data silos.
    We envision that this technology can reduce the impacts of public health crises by giving pharmacists, prescribers, and government agencies access to comprehensive, real-time information across the healthcare ecosystem.
     
  2. Support expanded roles for pharmacists.
    We want pharmacists to have access to a complete medication history so they can fulfill the full potential of their roles in preventing the spread of diseases, identifying potential substance abuse concerns, and identifying and filling gaps in care to underserved populations.

Integrate pharmacogenomic data into pharmacy benefit manager (PBM) systems and/or pharmacy benefit administrator modules and then push resulting clinical alerts to pharmacists, which will help speed patient access to the best medication therapy..

Positive Impact Potential

  1. Facilitate PGx testing for better medication matches.
    If a prescribed medication triggers a request that the patient complete genetic testing, the PGx test could be completed at the pharmacy or sent to the patient’s residence.
     
  2. Notify prescribers of test outcomes.
    Through NCPDP’s SCRIPT RxChange message, prescribers would be notified of more appropriate medications based on the PGx test outcomes.
     
  3. Reduce ER visits due to adverse drug events.
    If genetic data is stored in the PBM, the integrated flags and electronic prior authorizations will act as safety guardrails for patients.
     
  4. Reduce prescription waste.
    Using the NCPDP telecommunication standard, an alert within the PBM for highly adverse or ineffective medications will result in denials for future fills.
