Bridging Access to IBD Care Community Lunch & Learn

Join The Stephanie A. Wynn Foundation, Inc. for Bridging Access to IBD Care, a Community Health Partner Lunch & Learn bringing healthcare and community partners together to strengthen referral pathways and improve access to care for individuals living with Crohn’s Disease and Ulcerative Colitis (IBD).

Individuals living with IBD can face significant barriers when trying to access specialty care, supportive services, and trusted resources. This interactive Lunch & Learn will introduce attendees to the IBD Patient Navigator® Program and explore how healthcare and community organizations can work together to better connect individuals with the care and support they need.

Attendees will learn about common barriers to accessing IBD care, the role of patient navigation, how to refer individuals to the IBD Patient Navigator® Program, and opportunities to strengthen partnerships across South St. Petersburg.

Who Should Attend: Healthcare providers and practices, hospitals and health systems, social workers, care coordinators, community health organizations, public health professionals, patient advocates, and community-based organizations.
