Enrollment Withdrawal Form

The OCSA Withdrawal Form as the first step in the withdrawal process for your child. Your student's Withdrawal Form will be forwarded to the appropriate school departments and to the school Registrar for processing.

Concurrent enrollment in two primary schools is not permitted under state law. Your child must be exited from OCSA prior to beginning attendance at his/her new school.

The Registrar will contact the parent/guardian via email within 24 hours of receipt of the Form (not including weekends/holidays) with the next steps to withdraw.

NOTE
End of year and summer withdrawals do not require a meeting with the registrar.
School property must however be returned prior to the withdrawal.
