Sports Physicals Offered

Help promote sports physicals for us!

Parents can get their children’s sport physicals at the resource fair. Here's what they can bring/expect:

  • Payment: The first 25 sports physicals are FREE thanks to the Neighborhood Match Fund.
    After that, cash or card accepted.
  • Sports Physical Forms: Bring any school or league-required forms (make sure the parent/guardian section is filled out and signed in advance)
  • Eyeglasses or Contacts: If your child wears them, bring them for the vision screening
  • Duration of Validity: A sports physical is valid for one calendar year
  • Parent/Guardian: A parent or legal guardian will need to be present for minors, especially if consent forms are not pre-signed
  • Time Needed: The visit typically lasts 15–30 minutes
