Get your children’s sport physicals at the resource fair. Here is what to bring/expect:

Payment: Cash or card accepted (check if exact amount is needed)
Sports Physical Forms: Bring any school or league-required forms (make sure the parent/guardian section is filled out and signed in advance)
Eyeglasses or Contacts: If your child wears them, bring them for the vision screenin
Duration of Validity: A sports physical is valid for one calendar year (typically April to April)
Parent/Guardian: A parent or legal guardian may need to be present for minors, especially if consent forms are not pre-signed
Time Needed: The visit typically lasts 15–30 minutes
