The Family Health Partnership Clinic will improve the health status of the community and reduce the negative impact of poor access to care by providing a broad spectrum of services to the underserved with compassion and respect. We recognize that as a community, we are responsible for each other.
 

Our Vision

Over the next ten years, the Family Health Partnership Clinic will assure that the residents of the McHenry County area will enjoy the benefit of full access to high quality health care, regardless of the ability to pay. The Clinic operates on a sliding fee scale, although no patient is turned away due to inability to pay. At registration, patients will be asked to provide some proof of income.

A Brief History

In 1996, the Clinic began as a partnership with Community Health Partnership. While a primary focus was on the access to health care for migrant workers, it became quite clear that a larger and broader scope of services was needed. With cooperation from its parent agency, the Clinic became its own entity in 1999. Until the Clinic opened, there was no place in the county for people without health insurance except for the emergency room.

The Family Health Partnership Clinic Sage Center for Care, located in Crystal Lake, opened in November 2013 and in 2016, celebrated its 20th year.  A successful $3.2 million dollar campaign was championed through a generous lead gift from Vince & Pat Foglia and was led by a dedicated committee to raise the necessary funds. The new clinic is twice the size of the old clinic at 10,000 square feet and can serve twice as many patients and many more providers can volunteer than ever before. In 2017, a dental clinic and a mobile clinic van providing homeless healthcare was added to the clinic services. In July 2019, the Pink Door Thrift Shop was added to the clinic.  The upscale resale store has had a long-standing partnership with Centegra Health System for the past 39 years. The store’s mission has been to support local health care, and now will continue its legacy by supporting FHPC.

The clinic continues operation with foundations, grants, corporate, and community support as well as patient support from sliding scale fees. When it began, it was expected that the Clinic would see approximately 500 -1000 visits each year.  The Clinic now sees more than 625 patient visits each month and continues to grow with the generous support of the community. Patients and Providers

Medical Providers

FHPC's primary focus is to provide primary care for the uninsured.  We are fortunate to have a wide variety of doctors consistently volunteer their time and talents to provide that care.  Our doctors come from providers across the northern suburbs to care for our patients with the help of both paid and volunteer nurses.

Dental Providers

After the closure of the only dental clinic for the uninsured in the county, and through the work of Dr. Joe Hagenbruch and the McHenry County Dental Society, dental services began in 2017 and local dentists and hygienists volunteer their time to see patients.

Patients

There are many people who need the clinic.  In fact, we estimate that at least 20,000 people in McHenry County are uninsured.  Do you know someone who needs our services?  Please tell people you know—friends, coworkers, parishioners, neighbors, family—that the clinic is here for anyone who is uninsured in McHenry County.  Remember, no one is turned away for inability to pay.

Events and Fundraising

Spring Gala

Every year, FHPC hosts gala fundraiser and silent auction to raise money for needy patients in McHenry County.  The gala takes place in April, and each year brings a new and exciting theme.  This event features a variety of activities including the Fund-A-Need paddle raise, a wine pull, exclusive live auction items, and various raffles. Tickets are available for purchase to anyone who would like to attend, and mobile bidding is available for all silent auction items, even if you are not attending!

Care4 Breast Cancer 5K Run/Walk

The Care4 Breast Cancer 5K run/walk started in 2001, and in its first year hosted less than 300 people and raised about $5000.  It now takes place at Woodstock North High School and attracts almost 2000 people, both participants and volunteers, and raises almost $130,000 annually!  All net proceeds go toward the FHPC Breast Cancer Fund, which improves access to screening, mammograms, and treatment resources for the uninsured women and men of McHenry County, as well as increases public awareness of breast cancer issues specific to our county.
