For Bed Racing Teams

If the members of the bed racing teams are under 18 years of age, the parent(s) or guardian(s) must execute in addition to the above, the following waiver. For all minors, the completed youth waiver form must be emailed to elinore@flourishfurniturebank.org or dropped off in person at Flourish, 11971 Grandview Road, Suite A 9 am - 4 pm Monday - Saturday. Minors without a signed waiver form will not be allowed to race! Please note this is not required for attendees.
