For payroll deductions, check & cash access the following:

Contribution Form

Active Duty/Guard/ReservePlease fill in form on computer, digitally sign, and take one of the following actions:

  • For Payroll Deduction Plan contribution:  Email digitally signed form to your Squadron Key Worker, or if outside the campaign window, to usaf.fundraising@us.af.mil
  • For Check contribution:  print form (no SSAN or signature required) and mail with check/cashier’s check/money order to the address below.
  • For e-Giving contribution:  Just click on the red “DONATE TODAY!” button on the left side of this page. No separate contribution form is necessary!

RetireesPlease fill in form on computer (enter retired rank, not “CIV”), print form and take one of the following actions:

  • For Payroll Deduction Plan contribution:  Ensure full SSAN is entered; sign and deliver to your Squadron Key Worker if you are currently a Federal Civilian or contractor working for the DAF. If not, please mail to the address below. Payroll deductions begin 1 July and continue through the following year, 1 June.
  • For Check/cashier’s check/money order contribution:  Do not enter SSAN or sign the Contribution Form.  After printing, send with check to the address below.
  • For e-Giving Contribution: Just click on the red “DONATE TODAY!” button on the left side of this page. No separate contribution form is necessary!

** The built-in PDF viewer introduced with Mozilla Firefox and Google Chrome do not support XFA-based PDF forms. Therefore, XFA-based PDF forms do not open in later versions of Firefox and Chrome. XFA-based PDF forms and policy-protected documents require Adobe® Acrobat® or Adobe® Reader®, version 8 and later. See Adobe Acrobat Downloads for downloading the latest Reader or Acrobat. Once the installation process is complete, save the PDF document to your computer and open the file using Adobe Acrobat Reader (DC).
